Inspection Form

Throughout the Land Disturbance Process, inspection reports must be maintained by certified Erosion Prevention & Sediment Control Inspectors.

In the coming months, six newly permitted MS4 programs will begin implementation of the Construction Site Runoff Control and Post-Construction Stormwater Management Minimum Control Measures (MCMs) required in their permits. See important information here.

Applications and Forms that must be submitted are found in the first two sections below. These forms are used when applying for NPDES coverage for Industrial and Construction Activities associated with stormwater discharges.

All current Notice of Intents (NOI) are available for download below, along with their respective Notice of Terminations (NOT) and attachment forms to modify and transfer coverage.

Construction Activities

Notice of IntentForm D-2617 requests NPDES coverage under SC's CGP for Stormwater Discharges from Large and Small Construction Activities.
Transfer of Ownership
(NOI Attachment)
Form D-0434 is used to notify the Department of a transfer of ownership and/or the request for new NPDES coverage under SC's CGP for Stormwater Discharges from Large and Small Construction Activities for the new owner. This form is to be submitted with a complete NOI (d-2617) when requesting approval.
Major Modifications
(NOI Attachment)
Form D-0435 is used to notify the Department of a Major Modifications of the approved SWPPP and Construction Site Plans. This form is to be submitted with a complete NOI (d-2617) when requesting approval.
Notification
(Less Than 1 Acre)
Form D-2628 informs SCDES that any construction activities whose disturbed area is less than 1 acre, not part of a larger common plan, and outside the eight coastal counties will be conducted pursuant to State and Federal Stormwater Regulations.
Notification (Less than 1-Acre, Not Within 0.5 Miles of a Coastal Receiving Water) - Coastal Counties Only

Form D-0451 informs SCDES that any construction activities where disturbed area is less than 1 acre, not part of a larger common plan, not located within 0.5 miles of a coastal receiving water, and WITHIN the eight coastal counties will be conducted pursuant to State and Federal Stormwater Regulations.

Notification (Automatic Permit Coverage) - Coastal Counites Only

Form D-0451 informs SCDES that any construction activities where disturbed area is less than 1.0 acre, not part of a larger common plan, located within 0.5 miles of a coastal receiving water, and WITHIN the eight coastal counties will be conducted pursuant to State and Federal Stormwater Regulations.

Individual Lot Notice of IntentForm D-0432 requests NPDES coverage for a Secondary Permittee, that is a lot owner or builder, within a previously approved Residential Subdivision. request for coverage of multiple lots may be requested on a single application.
Annual Blanket Notice of IntentForm D-0433 requests coverage of registered utility providers or contractors performing construction-related activities within common developments (Larger Common Plans) under an approved SWPPP. This coverage can be used in lieu of providing a contractor certification
Contractor Certification FormForm D-0437 is a document to be signed by each contractor who will be performing work at an approved construction site. These forms are to be retained in the On-Site SWPPP and do not have to be submitted to the Department, unless requested
Pre-Construction Conference Certification FormForm D-0436 is a document to record that a Pre-Construction Conference was held, where it was held, who ran the conference, and who attended the conference. These forms are to be retained in the On-Site SWPPP and do not have to be submitted to the Department, unless requested.
Notice of TerminationForm D-2610 which should be completed to properly file for coverage termination under the Construction General Permit.
Expedited Review
Agreement Form
Form D-2618 is required agreement that must accompany all Expedited Reviews seeking coverage under the Construction General Permit.
Permanent Maintenance AgreementPermanent Maintenance Agreement which is required to be submitted with any construction activity that has permanent BMPs that manage stormwater.
Weekly Inspection Form (Template)A sample inspection from that can be used by Certified CEPSCI inspectors on any construction site currently covered under the NPDES Stormwater Program.
Consolidated Permits
Program (EPA)
EPA's Application Form 1 that contains general information about the applicant and facility. This must accompany the individual NPDES permit application - Form 2f.
Form 2F (EPA - Industrial)EPA Application for individual NPDES permit coverage. Only required to be submitted when SCDES deems appropriate.

Industrial Activities

(Starting July 1, 2022 all applications must be requested through ePermitting.)
